Our team of receptionists want to help you. They need to know your:
- Full Name
- Manitoba Health Number
- PHIN Number
- Contact Information
- Nature of your visit.
Please let them know if your visit is for an urgent or non-urgent problem, a follow-up visit, maternity care, a complete physical, insurance physical, or driver’s physical.
This information enables the receptionist to book an appropriate appointment for you. Medical forms to be filled out should be shown to the receptionist as some information on the form may have to be filled out by them before seeing the doctor.
NO SHOW FEE
Please provide 24 hours notice, where possible, to cancel an appointment. We reserve the right to charge a NO SHOW fee of $40.00 for missed appointments.
